Completing The Medical Travel Form For My Upcoming Surgery Physiotherapy Rehab Facility

I am really fortunate to have a family doctor. I am very grateful to the local hospital emergency department doctor who stuck his neck out for me in order to make this happen. I have just had the second appointment with my family doctor. He has completed the Ontario Disability Support Program form that designates the warm water therapy pool operated by the city where I live as part of my medical treatment. The next step is for me to scan this and submit it online to my case worker. I have already discused this with my case worker. I have been assured this facility will be approved for reimbursing the travel expense for me to make use of the warm water therapy pool during my recovery from knee replacement surgery and if this is medically warranted once I have healed to manage the needs of my physical disability. I am very grateful for the people who believe in me and the resources being invested in me to undergo my right knee joint replacement surgery and subsequent recovery.
